Dr Cai studied medicine at the University of New South Wales where he graduated with first class honours. He obtained a Masters in Internal Medicine from the University of Edinburgh and was awarded a Distinction for his thesis. His medical training includes internship and residency at Liverpool Hospital, physician training at Royal Prince Alfred Hospital and rheumatology training at Royal Prince Alfred and St George Hospitals. Dr Cai was awarded the prestigious Leanne Stafford Award from Arthritis Australia to fund a research fellowship in New Zealand. He is currently completing a Doctorate of Medicine exploring the relationship between gout and cardiovascular disease. Dr Cai is a staff specialist at Westmead Hospital where he is the clinical lead of the Osteoarthritis Chronic Care Program for Western Sydney Local Health District.

Dr Cai has presented his research at local, national and international rheumatology conferences and has published extensively within the field. He is the principal investigator on multiple clinical trials and a research supervisor for the University of Sydney. Areas of research include crystal arthritis, inflammatory arthritis, cardiovascular disease in rheumatology, the development of core domain sets for rheumatology trials and the use of large datasets for medical research. He is actively involved in the education of junior doctors and medical students and is a Clinical Senior Lecturer for the University of Sydney. Dr Cai is a member of the Australian Rheumatology Association, the American College of Rheumatology, the Gout and Crystal Arthritis Network and OMERACT (Outcome Measures in Rheumatology).
